 Objective: This study aimed to investigate the efficacy of metacognitive therapy (MCT) on distress tolerance, executive functions, and substance use tendency among aggressive adolescents. Method: This quasi-experimental study employed a pretest-posttest design with a control group. The statistical population comprised all male lower-secondary school students enrolled in the 2024–2025 academic year at Imam Mohammad Baqir School in Jask County. Using convenience sampling based on cut-off scores from the Ahvaz Aggression Questionnaire, 30 students identified as aggressive were selected and randomly assigned to an experimental group (n = 15) and a control group (n = 15). For data collection, participants completed the Ahvaz aggression inventory, the distress tolerance scale, the executive function questionnaire, and the addiction proneness scale. The experimental group received Wells' MCT protocol across 10 weekly sessions (1 hour each), whereas the control group received no intervention. Data were analyzed using multivariate analysis of covariance. Results: The findings revealed a statistically significant difference between the experimental and control groups. Specifically, compared to the control group, MCT significantly increased distress tolerance, improved executive functions, and reduced substance use tendency in the experimental group at posttest. Conclusion: The results indicate that MCT can serve as a targeted therapeutic strategy to attenuate risk-taking behaviors and enhance self-regulation in aggressive adolescents. These findings underscore the necessity of integrating metacognitive programs into psychosocial interventions for aggressive adolescents, particularly within educational and clinical settings.
