Objective: The present study aimed to investigate the effectiveness of cognitive-behavioral therapy based on Marlatt's model on attention performance and relapse self-efficacy in patients undergoing methadone maintenance treatment. Method: This was a quasi-experimental study with a pre-test, post-test, and two-month follow-up design with a control group. The statistical population consisted of all male opium users who referred to the Avayeh Zehn addiction treatment center in Tehran in 2022. Based on the inclusion criteria and using G*Power software, 40 individuals were purposively selected and randomly assigned to either the experimental group (n = 20) or the control group (n = 20). Data was collected using a researcher-made demographic checklist, the computerized Stroop test, and the relapse self-efficacy questionnaire. The experimental group participated in 12 sessions of treatment (one 75-minute session per week) based on the Marlatt protocol. The collected data was analyzed using repeated measures analysis of variance. Results: The total scores for attention performance and relapse self-efficacy in the experimental group significantly improved compared to the control group, which remained stable during the follow-up period. Conclusion: Marlatt-based cognitive-behavioral therapy can be used as an effective intervention to improve behavioral status and attention function in individuals with substance use.
