Objective: The present study aimed to determine the effect of cognitive-behavioral therapy based on Marlatt's model on craving and addiction quitting self-efficacy in patients under treatment with methadone. Method: This study was semi-experimental with pretest-posttest design with a control group. The statistical population included all patients under treatment with methadone referring to Mahan center of Bojnourd city in 2021. Among them, 30 people were selected by convenient sampling method and randomly assigned to experimental (N=15) and control (N=15) groups. The instruments used in this study were the periodical craving questionnaire and the quitting self-efficacy questionnaire. Cognitive behavioral therapy based on Marlatt's model was performed on the experimental group for twelve 90-minute sessions with a frequency of two sessions per week. Data were analyzed by multivariate analysis of covariance. Results: The results of multivariate analysis of covariance showed that there was a significant difference between the two groups, and cognitive behavioral therapy based on Marlatt's model reduced craving and increase the quitting self-efficacy in patients under treatment with methadone. Conclusion: It can be concluded that the cognitive-behavioral program based on Marlatt's model can help improve the effectiveness of methadone therapy while improving the capabilities of patients and play a preventive role in disease relapse.
