بررسی تاثیر آموزش شناختی- رفتاری بر کاهش میزان افسردگی معتادین مقیم در مرکز اجتماع درمان مدار (T.C) تهران

طرح مساله: افسردگی یکی از علایم شایع در بسیاری از معتادین به مواد میباشد که در زمان اعتیاد و دوران ترک برای آنها آزار دهنده بوده و وجود آن حتی هنگام ترک نیز ممکن است منجر به عود مصرف مواد گردد. بر این اساس پژوهش حاضر با هدف بررسی اثربخشی آموزش شناختی- رفتاری در کاهش علائم افسردگی معتادین اقامتگاهی حاضر در مرکز اجتماع درمان مدار تهران در سال 1385 انجام گرفت.
روش: این پژوهش به روش نیمه تجربی و طرح پیش آزمون- پس آزمون با گروه کنترل بر روی نمونه40 نفری از معتادین مرکز اجتماع درمان مدار هجرت (T.C) تهران که با روش نمونه گیری تصادفی و همتاسازی انتخاب شده بودند اجرا شد .ابزار اندازه گیری، شامل پرسشنامه 21 سوالی افسردگی بک (1978) بود.. داده های حاصله با روش های آمار توصیفی شامل میانگین و انحراف استاندارد و روشهای آمار استنباطی، آزمون T استیودنت همبسته برای مقایسه درون گروهی، و آزمون T استیودنت مستقل برای مقایسه بین گروهی در نرم افزار SPSS مورد تجزیه و تحلیل قرار گرفت.
یافتهها: مقایسه نمرات پس آزمون افسردگی در دو گروه آزمایش و کنترل (20/T=4) (P<0/001) بیانگر بوجود آمدن تفاوت معنادار نمرات افسردگی در بین دو گروه بود در حالیکه نمرات پیش آزمون دو گروه تفاوت معنی داری را نشان نداده بود. بدین شرح که گروه دریافت کننده مداخله شناختی- رفتاری کاهش معنی داری در شدت علائم افسردگی، نسبت به پیش آزمون و گروه کنترل که مداخله ویژه ای برای نشانه های افسردگی دریافت نکرده بودند، نشان داد.
نتیجهگیری: تفسیر این نتیجه بدین معنی است که مداخله روانشناختی با رویکرد شناختی – رفتاری برای مقابله با افسردگی نقش مهمی در کاهش افسردگی معتادان مقیم در مرکز اجتماع درمان مدار دارد. بنابراین از این طریق می توان یکی از عوامل خطر عود مصرف را که همان احساس افسردگی می باشد بر طرف نموده و موفقیت بیشتری در جهت رهایی از اعتیاد حاصل نمود.
Objectives: Depression is one of the prevalent symptoms in addicts that are aversive during the dependency and withdrawal period, even its presence during recovery can lead to relapse. So, the present study was conducted to compare the effectiveness of Cognitive Behavioral Therapy (CBT) on depression reduction in Tehran therapeutic community center resident addicts in 1385 according to this magnitude effect.
Methods: Design of this research was pre-experimental with pretest- posttest design by control group. Instruments: Initial assessment was made on all of center residents with using the Beck questioner (1978), and analyzed with descriptive and inferential statistic in SPSS. Afterwards 40 self-referred addicts who had depressive criteria were randomly selected to take part in this study. The subjects were randomly placed in two experimental (CBT, Control) groups. The experimental group received sixteen weekly sessions cognitive-behavioral training. Then post-test assessments were made.
Finding: Findings of this research clearly showed the effectiveness of the cognitive-behavioral interventions in depressive symptoms reduction. So, when compared to the pre-test and control group which did not receive any interventions for the symptoms of depression, the group that received CBT did not show significant reduction in the intensity of symptoms.
Discussion: The results show that psychological interventions in cognitive behavioral approach played a very crucial role in reducing depression in the addict's resident at the therapeutic community. Therefore, depression, that is one of the relapse risk factors, could be obviated and more success gained.
