Objectives: Depression is one of the prevalent symptoms in addicts that are aversive during the dependency and withdrawal period, even its presence during recovery can lead to relapse. So, the present study was conducted to compare the effectiveness of Cognitive Behavioral Therapy (CBT) on depression reduction in Tehran therapeutic community center resident addicts in 1385 according to this magnitude effect.
Methods: Design of this research was pre-experimental with pretest- posttest design by control group. Instruments: Initial assessment was made on all of center residents with using the Beck questioner (1978), and analyzed with descriptive and inferential statistic in SPSS. Afterwards 40 self-referred addicts who had depressive criteria were randomly selected to take part in this study. The subjects were randomly placed in two experimental (CBT, Control) groups. The experimental group received sixteen weekly sessions cognitive-behavioral training. Then post-test assessments were made.
Finding: Findings of this research clearly showed the effectiveness of the cognitive-behavioral interventions in depressive symptoms reduction. So, when compared to the pre-test and control group which did not receive any interventions for the symptoms of depression, the group that received CBT did not show significant reduction in the intensity of symptoms.
Discussion: The results show that psychological interventions in cognitive behavioral approach played a very crucial role in reducing depression in the addict's resident at the therapeutic community. Therefore, depression, that is one of the relapse risk factors, could be obviated and more success gained.

Harm reduction measures in prison (Methadone treatment) and its effects on quality of life promotion
Iraj Esmaili
تهرانiesmaili@yahoo.com
1
Preface:Most of the people, who are incarcerated, are considered as competent labor force and in the most of the times they are young people of the society and they would return to the community after serving a period of conviction. Promoting health condition in prison population and effective use of imprisonment period is a safe way to rehabilitate the prisoners. Harm reduction programs begin with supporting “a group of people who have risky behaviors or can not quit drug abuse or do not like give them up”.
The philosophy of harm reduction is based on pragmatism theory. In fact harm reduction paradigm is focused on the individual risky behaviors reduction. The practical principles of this paradigm are based on empowerment of risky personalities. Harm reduction defines the pattern of substance abuse as a range, from negative consequences for abusers and society to abstinence.
Method: The first part of this six months study was cross-sectional and the second phase of it has been conducted as a longitudinal survey in six months period. To study the objectives and to collect the data, a demographic questionnaire andSF-36 questionnaire has been used.
Finding:In three different prisons Evin (27 person 10.8%), Rajaeeshahr (88 person 35.2%) and Ghezelhesar (135 person 54.0%) were selected. According to the data the prisoners who received the Methadone treatment during this 6 months, have been improved in Physical Function (p<0.010), Bodily Pain (p<0.003), Mental Health (p<0.019), Vitality (p<0.001), General Health (p<0.017) and Social Function (p<0.041) dimensions quality of life status significantly. There were not seen any improvement in Role Physical (p<0.0710 and Role Emotional (p<0.313) dimensions.
Conclusion: In a look, it is obvious that the harm reduction measures which have been taken for prisoners under the coverage of Methadone treatment will be effective in improving their quality of life conditions.

Sprcial discussion: Future Roles for Tincture of Opium in Medical Intervention for Opioid Dependence Treads and Hopes
h.ekhtiari@gmail.com
1
Opium tincture or tincture of opium (TOP), also known as Laudanum, is an alcoholic herbal preparation of opium. It is made by combining ethanol with opium latex or powder. After works of Paracelsus, the 16th century Swiss-German alchemist, who discovered that the alkaloids in opium are far more soluble in alcohol compared to water and named Opium tincture as Laudanum, this drug has been found a broad range of applications as the drug of choice for practically every ailment through 17th to 19th centuries. The early 20th century brought increased regulation of all manner of narcotics, including laudanum, as the addictive properties of opium became more widely understood. By the late 20th century, TOP's use was almost exclusively confined to treating severe diarrhea. During these thirty years, The efficacy and cost-effectiveness of opioid substitution therapy for the treatment of opioid addiction has been well documented by methadone and buprenorphine. Based on the idea of substitution therapy, TOP and slow releasing oral morphine (SROM) have been proposed as the new alternatives. TOP could be used in three different ways in interventions for opioid dependency, first, as a supportive drug to reduce withdrawal syndrome during detoxification programs, second, as a substitution drug in long term maintenance, third, as a legal form of opioid drugs for harm reduction purposes. A little but growing evidences are supporting effectiveness of TOP for detoxification programs, in Iran, some preliminary open label studies showed effectiveness of TOP as a detoxifying drug in three to six month program, Now another open label study is evaluating TOP in a specific detoxification program with monthly evaluation and providing long term follow up after reaching to abstinence. There are a strong debate among the policy makers and clinicians on implementation of TOP in Iranian National Substance Abuse Treatment Facilities. In this article, We reviewed the proposed threads and hopes that is expected for TOP in Iran and other countries with the same situations.
